As per new guidelines issued by Ministry of External Affairs in December, 2016, RPOs in India should not insist on duly attested affidavit from parent living abroad and should accept application for issue of passport to minor child on basis on affidavit given by the parent residing in India ( see link to annexure for appropriate annexure).  However, if RPO still insist on production of affidavit, it can be issued on request basis. Requisite documents are:

• Duly CompletedEAP 2 Form- click here   .
• 1 recent photograph with size ( 5 cm x 5 cm) -.015h0l white background  
• Duly filled in Annexure ‘D’ – click here for Click Here. 
• Original passport and its photocopy (first and last page, valid visa page).
• Photocopy of the passport or birth certificate of child

First get Approval of Affidavit at High Commission of India from 9.30am to 12.00pm (Monday to Friday) and need to do payment at BLS
